what it is: Incentive program enables high- risk Medicaid members—pregnant women, new moms and patients with diabetes or hypertension—to earn up to $100 for completing speci c healthy activities.
why it works: The card is combined with

to improve member health. Card restricts spending at the UPC level and provides dis- counts on healthy products.
